Thank you for helping us recognise people making a difference in our community.

You may find these questions helpful when preparing your story. You do not need to answer all of them. To make the process easier, we recommend preparing your story before starting the form and uploading it as an attachment (Word document or PDF).

Your story does not need to be formal or extraordinary. Everyday moments and meaningful contributions matter too.  You may want to consider some of the questions below when preparing your story. You do not need to answer all of them.

• What have they done to make a positive impact?
• Who has benefited from their work or contribution?
• Can you share a specific moment, story, or example that shows their impact?
• What makes their contribution special or meaningful to you or the community?
• Is there a small everyday action or contribution you would like to recognise?
• From your knowledge, what type of recognition would feel most meaningful or comfortable for this person or group, whether that is public recognition or a more personal acknowledgement?

Please avoid including personal contact details of the person or group being recognised at this stage. If additional information is needed, we may contact you directly.
